Needs 1.3.14+ for This one on Ongoing (1.3.3+ for callout/quote). After updating, click Reload on chrome://extensions (Load unpacked alone can leave the old worker). Under Ongoing heading, checkboxes, plain lines, bullets, or callout/quote text all count.

No. Notion's free plan works. Use a bare page per project with a simple to-do list. A Standing or Ongoing heading on that bare page still works on free. No fancy databases.

Under a Standing / Ongoing heading, use checkbox to-dos or plain lines / bullets. Outside that heading, use a checkbox starting with Ongoing: or ∞. Callouts and quotes under Ongoing count too. Needs extension 1.3.8+ for This one (site ships 1.3.22).
No. You cannot check them off from the app. Done on an Ongoing hero only clears local focus. Edit or check in Notion if you truly want them done.
Yes. Your normal to-dos outside that heading / prefix still work as finishable one-things.
